Siaya County has Kenya's highest HIV prevalence rate, at over 20 percent in some areas. The reasons are structural: fishing communities with high mobility, transactional sex along trade routes, cultural practices around widowhood and cleansing rituals, and decades of inadequate health infrastructure. The crisis reshaped family structures, orphaned a generation, and made HIV testing and treatment a defining feature of life in Nyanza. The response combined medical intervention with cultural negotiation, challenging traditions while building community-led care systems. The epidemic is not over, but Siaya has learned to live with it.
