The Kenya Demographic and Health Surveys document socioeconomic and demographic factors associated with under-five mortality, revealing substantial inequalities across wealth quintiles, education levels, and residential areas. Children in poorest households and rural areas experience 2-3 times higher mortality than wealthiest urban children, reflecting gaps in healthcare access, nutrition, and water/sanitation infrastructure. Parental education, particularly maternal education, strongly predicts child survival, reflecting higher health knowledge and care-seeking among educated mothers. These disparities require targeted investments addressing not only health services but also underlying poverty, education, and infrastructure gaps.

Multifaceted pediatric care quality improvement initiatives at district hospitals have demonstrated cost-effectiveness in reducing childhood mortality. Training health workers in standardized protocols, improving essential medicine availability, and monitoring treatment outcomes enable progressive improvement in care quality. Community health worker programs extending child health services to households identify seriously ill children requiring facility referral. Strengthening referral systems connecting community and facility levels enables appropriate treatment at levels capable of managing illness severity. Kenya's success in reducing under-five mortality demonstrates potential for further improvements through continued investments in pediatric health service quality and health system functioning.
